A wizard who won't cast any spell other than magic missile.
In a one-shot that I played in a while back one of the players started out with a character that they fully expected to die so they prepared a back-up character. It was a dehydrated pixie (like a sea monkey) that someone just had to drop into water once her old character died and in a little while it would be a full grown pixie. Keep in mind that the entire camapign was meant as a joke.

Carry an Umber Hulk and a Purple Worm (both polymorphed into pebbles) in your belt pouch. If things get rough, toss them on the floor and have the party mage dispell the polymorph. This usually generates about the same energy level as tossing a live squirrel into a room full of retrievers. The Umber Hulk will try to eat the Worm, while the Worm will lash out at everyone. Makes for great escape cover.
Variant of same tactic, carry around about a dozen pebbled Rust Monsters. Toss the pebbles behind the enemy, then dispell. The enemy is usually laughing too hard at the "missed" spell to notice what's going on till they're half naked.
I found these both freaky, the first when our party pulled it on the bad guys, the second time when a group of NPC's pulled in on US!

Pulling that off would be a DM masterstroke.

A giant metal horses rear complete with legs. (Some wise ass contruct crafter was told to make an iron golem in the Counts image). guess where the poisonous breath weapon comes out!
A zombie that eats itself and then regenerates new body parts every midnight.
A squirrel that explodes in a 10d6 fireball. No reason why or how, and it never happens again.
A druid with an invisible horse animal companion. (It's really just a magic flying saddle and he hasn't figured it out)
A town crazy that only speaks in eight word sentances and one of the nouns is always the opposite of what he intends to say. Oh, and he sees the future.
A larval stage monster that infects a host like a disease and grows inside the eyeball, making the victim blind and eventually blowing out the eyeball to escape.
A medusa that makes her lair in a special mirrored labyrinth. Special because the mirrors are magic and her image in the mirror conveys her gaze attack, and the angles of the mirrors change to cast her image as frequently as possible no matter where she goes in her home.
A creature that is born old and crippled but gets younger and stronger as time passes. Once it gets to be the equivalent to a young adolescent stage it's life cycle is completed. It dies and three old crone monsters crawl from its carcass.

A wisard/ Psion/ archivist who only attacks with his weapon forgeting that he has spells then finds out that his spells are really pointless to have
And when he uses his spells he only uses the wimpy ones. mostly magic missle. When he levels up he refuses to take some serious kick ass spells like fireball or whatever.

well, for a decade we have joked and laughed about a trollblooded, vampiric, demonic, were gecko of death (from geckoslavakia no less); but now in this game; you can really build one. That is pretty freaky, fear the gecko.
Unless said demonic gecko is trying to save you money on your insurence.

Have a big island (or continent) that has never had humans on it. Like only yakfolk or rackshasa societies.
A spell (like heat metal) that goes awry and turns all iron (for a few miles radius) into softer metal so, for a time, metal weapons and armor are rendered useless. (Imagine this happenning in a war scenario)
This always creeps players out... The PCs arrive in a town (one that they've already visited) that is completely deserted. All the inhabitants and wildlife are gone. This could remain unexplained until the players reach higher levels.
An intelligent sword with an ego that manifests itself as an NPC only visible to the weilder. This NPC could be a longtime friend of a PC without him even knowing that he is the only on seeing him (telling him to do things / being always around when he needs him).

There is a character that is a freak in one of my games; he was a guy that made a deal with his dark god and got changed into a female and she is always pregnant and gives birth to some kind of monster every few months; she is kinda marsupial; she has the demon; aberation, and darkness templates and calls herself the Mother of Darkness, LN but i cannot describe some of the things that crawl ahem and call her mother before they are weaned and go off to do the dark gods bidding; so it is kinda like a pet class whereas all of her pet childen have the dark template. This is only the start of how this guy is a freak; he spend quite a few levels eating demons in the abyss and those on the prime material as they turned to goo; he would even slurp the goo; so he is infested with demon maggots in his stomach and gained the ability to make a acid attack breath weapon from stomach acid loaded with demon maggots; the maggots then crawl back to him after damageing or eating a foe and crawl up his/her body and through mouth or nose back home to the belly. I could keep going; this is only the surface of this guy's pc; he is playing the biggest freak in my game ever. But, is a fun character and has loads of storyline threads and enemies and followers; the character is about 17th level atm.

A froghemoth storms past the party in the wilderness, ignoring them totally. If the party is curious enough to follow,...they come to a large tidepool. A female froghemoth has laid hundreds of thousands of eggs, 100 yards by 50 yards of eggs. There's 8 male froghemoths fertilizing the eggs, oblivious to the party...

Doing it. Focused Transmuter/Fighter/Eldritch Knight with banned schools of Evocation, Enchantment, and Necromancy. I may not be able to cast Fireball, but I have buff, debuff, and battlefield control spells out the wazoo -- Slow + Evard's Black Tentacles lets me stand back and smack the poor buggers with my Glaive; Mirror Image, Invisibility, Freedom of Movement and Fly give me all sorts of options if I decide I want to not get hurt... and let's not forget the versatility in relegating my bonus spell slots to Rary's Mnemonic Enhancer, or Telekinesis.
Granted, I'm coming into a group that has a Dragon Disciple and an Arcane Trickster who are both focused on the "blowing shit up" aspect of arcane magic, so it's not like the group is actually losing anything.

The LN cleric of Pelor would be a freaky thing in a game. A dogmatic, non-benevolent party healer. Sounds like Mr. Shiny's current character.

In one of my homebrew worlds that was focused on an archipeligo, I made an entire nation of skelitons. These folk were undead in that they had undead traits, but could level in PC classes and were for all other intents and purposes just like elves, humans or other intelligent races. The animals on their island were likewise skelital and the arcitecture they used was much different than found among the "normal" races. They were expert sailors and chose piracy as often as they chose merchantilism and were fearsome to fight as well as negotiate with, when they even deemed you worthy of their conversation. They considered all of the "fleshed" races inferior, fit only to plunder or use to their advantage in business dealings.
Given their immortality, lack of a need to sleep, and inability to become fatigued when rowing and sailing, they were a fearsome and respected race. I forget what they called themselves, but their society was known as the "Empire of Bone".
