You were all aware of each other, so no true Surprise Round. I suppose he was making a move action to get out of the Oubliette, and so you could also have a surprise round action, such as casting Gravity Bow, and stowing the crossbow in Anna's case. EDIT: Oh yes, my "Ring 2" comment was for the movie The Ring 2, when the evil dead girl is climbing out of the well like a freakish spider.

Round 1
Tolvan pulls a magic arrow from his quiver (Undead Bane) and fires it at the creature to incredible effect, striking the Lopper in the chest and tearing a grisly chunk of psuedo-flesh and unholy material from his body. It shrieks and fixes it's gaze on the nearby wizard. "The creature is incorporeal and will require magic to hit it. Andjela, use the axe." he recommends.
Anna stows her crossbow, heading Tolvans warning that it will do no good. She pulls out a flask of holy water and unstops it, readying it in case the Lopper gets close.
"Protection would be good!" Andjela drops her curved-blade and reveals Lopper's Axe. You lookin' for this, b**ch-boy? Come and get it." The paladin readies herself for any aggressive move from the Lopper.
Kimberly, "Pharasma bless these heroes to send these lost souls to the afterlife" Kimberly continues her prayer in celestial and a wave of Pharasma's warmth engulfs the party.
Andjela, I wasn't sure if your readied action included moving to attack or not, but it makes more sense to me that you would move to attack if the Lopper was aggressive, so here we go.
Seeing the wraith move to attack Danica, Andjela steps forward to attack with the cursed, blood-covered axe. The blade strikes through the wraith, eliciting a ghastly shriek.
The Lopper lifts into the air, eying the axe in Andjela's hand with open malevolence. It glides towards the nearby monk, carefully avoiding the axe. The phantom axe it wields cuts through the air towards Danica, hitting and passing through her body like so much smoke. However several deep and heavily bleeding cuts appear on her shoulder and continue to bleed out Danica's life-blood. Instead of spilling to the ground, Danica's blood instead turns to smoke before your eyes and drifts toward the maniacal wraith floating in the air. The smokey blood seems to rejuvenate it somehow, and it cackles with evil glee. Danica: -3hp and -5hp in bleed damage. This -5hp will continue each round as per **normal bleed damage** rules.
Danica's rage burns at the sight of the Lopper. She begins to cut with her sword but feint instead, revealing the flaming fist that she showed once before ready to make contact. Her flaming fist deals the specter a devastating blow, and the Lopper is visibly rocked by the damage deal, it's eyes tracking the flame with something akin to fear. It visibly shudders and writhes, as if reliving a distant, horrible memory. Staggared
Active Effects: Bless - Duration 2 min. all allies gain a +1 morale bonus on attack rolls and on saving throws against fear effects.

I think Danica was flat footed when the Lopper attacked her last round, but I believe her Mage Armor should count as full 4 points of armor against its incorporeal attacks, so this round she should be at her full AC.

Can't see the map from work so can't see how everyone is positioned.
The monk drops her sword and clenches her new wound with her free hand in order to stop the bleeding. I better stop this before I bleed out. Danica reaches for a potion.
She moves around the Looper in order to give Andjela flanking advantage and enters into a defensive position. The monk continues to show her once flaming fist now slightly smoking.
Actions: Potion of CLW, enter into defensive position (no attack)

Good catch on the init change Andjela, forgot to mark that one. Also Danica, why don't you list an alternative action in case Kimberly decides to heal you. Or you can just leave it as written regardless, up to you. I intend to post Round 2 as quickly as I can once Kimberly posts, so you have that long to decide, fyi.

Both retrieving a potion and drinking it will provoke an attack Danica. And a good attack might drop you. I'd say it will drop you if you are hit again first, as the lopper goes before you. Best to wait til Kimberly and the Lopper goes before deciding what to do.

At whatever point, during combat or after, you attempt to release and stow away the axe, you find yourself unable to release it from your fingers. It has been fused to your hand with some other-wordly power, and your hand cannot relinquish it from your grip, asleep, or awake, under any circumstances. It's perpetually bloody surface feeling oily and slightly slick under your fingers. This inability to relinquish the axe is the only manifestation of its curse... so far.

Anna steps up to the apparition, her face a mask of fear, but she had to get close to douse the holy water on it. She wished she could come up with a quip. The holy water isn't thrown well enough to hit the wraith full on, but it is splashed by a portion of the holy liquid, it's body puffing into smoke where the water hits it. It gnashes it's teeth and howls an animalistic sound at the rogue.
Kimberly sings a short tune in celestial and then advances to try and safely touch Dancia hopeing to quell the bleeding. She keeps to the walls and ends up behind Danica, touching and healing the woman with her god-bestowed power. The moving around getting her nice clothes all dirty. The deep cuts on Danica's shoulder close nearly all the way, and the bleeding stops completely. +7hp Danica, bleed damage stopped.
Andjela continues to call on the evil wraith, "Come, face retribution... Scum!" The Lopper hisses at her in fury as the axe slices again through it's ghostly form, and Andjela can somehow tell it's doing more damage than is apparent. Indeed, as the axe finishes its swing, the Lopper stops moving completely, a look of surprise taking over the grotesque anger. It looks at Andjela and then the rest of you for the space of a heartbeat, and then it's body slowly dissolves into a gray smoke which drifts apart and into nothingness.
Combat is over! 320xp each.
The grate to the Oubliette, suspended on the ceiling the entire fight, slams back down to the floor, but at an angle, and can be easily shoved aside. As you look over the edge,the walls of the oubliette are polished smooth with very few handholds— it'd take an extremely skilled climber to make it down without hurting themselves. If Tolvan directs his light down there, you can see some skeletal remain in a heap at the bottom of the oubliette, along with about 30 feet of old rope. In addition, you can see a fair amount of gear and other treasures -- probably looted from murdered guards or stolen from guard rooms-- lies among the Lopper’s broken and mishapen bones. Oddly there is also a skull in there, contained inside a guards helmet, the rest of the body is not in the oubliette.

Anna looks bewildered for a moment. We got him! Ha, we got him! She glances around, but can't help but breathe a tentative sigh of relief.
Glancing down, Anna mutters, Not getting down there except the easy way. Looks like there might be some neat stuff, though. How about we get some rope?

Tolvan breaths a sigh of relieve. Good teamwork everyone. That could have been much more deadly if more of us were hit and bleeding. My rope is hanging at the entrance to this level. Thinking about options, he asks Before we climb down, lets go back and open that door we passed while our buffs are running. Bless and Gravity Bow
That did go better than I expected. I used one of the two Undead bane arrows but I think that was a good use of it. And I uses one of the 4 remaining ghost touch arrows too. So down to 1 Undead Bane, and 3 ghost touch, and 9 +1 arrows. GM Mug, how close are we to leveling?

Yeah you guys rocked him, especially that first arrow of yours Tolvan. And Danica picking up on his dislike for fire helped. And his axe Andjela used caused him an additional 1d6 which I rolled and it rolled a 6 and a 5 for his two hits. If he could have gotten a few of you to bleed, he'd have been quite the handful. You're at 4520 / 5000 as far as leveling is concerned.... but who's counting? :)

The door to this guard room is closed tightly, a metal object broken off in the lock. This door looks like it'd need to be forced open.

Let's go get the goodies. There's no more evil down there, right? The three of you can lower me down and pull me back up. Anna takes one end of the rope Danica offered and ties a loop. Then she threads it through her belt and puts her foot through the loop. This shouldn't take more than a couple minutes. She makes sure there's room in her backpack.
