The team breaks through to the tunnel below. It is dark and ten foot wide, and of round features, almost as if something had eaten its way through the mountains...
Some spots along the walls show signs of chemical reactions between the rock and an unknown substance.

Talan has been quiet while investigation and tearing up the floor occurred, and one hopes he remains quiet now, since he seems largely uncertain what to make of the tunnels around them or tracking the path of their prey.

Silgil, as one would expect from a dwarf, finds little difficulty keeping you on track as you wind through the interconnecting tunnels. After a (relatively) short time, you emerge once more to the blasted heat of the mountains.
Two tall sandstone outcroppings rise to the north and south, channeling whistling wind and stinging sand through this pass. The carcass of a giant serpentine beast covered in dried blisters lies in a heap at the gorge’s narrowest point, the body savaged by some powerful predator.

So, are we headed towards the cliffs? Also, is the Cave exit where we would have exited had we stuck to the caves instead of using the worm tunnel?

You have no idea about the other cave exit and where it may lead. You can all make perception checks though
A strong wind blows dust and dirt around the valley, making visibility difficulty.

As you advance down the valley there is a massive rockside. While you manage to avoid it, the massive corpse is in the middle of its path.
Many of the blisters on the body burst open and a dry powder flies into the air, choking the team.
Meanwhile a massive, draconic looking creature looms up at the top of the cliff.
"You dare trespass on My domain a second time!
It takes a huge breath in.

Most of the cliffs are high and crumbly (expect a climb DC between 20 and 25). There is a collapsed bit of cliff that looks quite a bit easier (climb DC in the 15-20 region) - that is to the north east. (top left corner)

Talan gags on the spores pluming out of the corpse, but a several good mouthfuls of spit and he doesn't seem worse for the wear beyond watery eyes.
He move away from the sporish area, drawing on his own talent and a mix of the sage jewel's power to generate an obscuring mist at his new location.

Green cross is the edge of the poisonous cloud that is quickly dissipating, however there are still loads of unpopped blisters on that body...
Zax drives a crosshow into a wing joint. It goes straight through, narrowly missing something vital.

Those rocks are much harder to climb than Zax was expecting. Taking that blast full on probably didn't help either.
But on the plus side, with a roll that bad, you didn't get high enough up to fall down.
Remember there is a mythic ability that can lessen the amount of damage you take, and also gain some damage reduction in upcoming rounds.

The cliff is 50' tall, but the dragon like creature is clinging to the cliff half way down. So it is only about 20' up.

Zax thinks with his heavy armour, it is going to be almost impossible to climb that rockslide.
Silgil doesn't have a clue what the creature is. It certainly looks like a dragon, but he can't recall hearing about them holding onto cliff faces.

Talan was staring at the monster through the mist, trying to identify it. It's tough to say if he knows anything about it, the fog kept getting in his eyes.
And then he got distracted, and wound up spending several moments just staring at Zax, utterly deadpan expression and a slight twitch dominating his face.

The sorcerer taps into the sage magic fueling him and brings forth a new spell, causing magical grease to smear on the cliff face beneath the dragon thing.
Standard to cast grease, moving back into the fog to hide afterwards.

If you have a way to reach 20' up the cliff, you can attack it with a melee weapon. Most folk can easily strike 10' up. With an enlarge you could get to 20 probably. Add in a reach weapon and you'd be able to make it easily.
Talan has no idea what the creature is.
