When I DMed a Rise of the Runelords campaign several years ago, Mokmurian lived. It is a long, convoluted story -- his survival coinciding with the PCs' victory, but at the end he lived.
As a result, I've included him as a living and obviously quite powerful NPC in two other campaigns in Varisia where his presence, in periphery, made sense. The ultimate conclusion of which is that, in my Pathfinder world, Mokmurian is alive and well,... and powerful.
That brings us to Volstus.
A badass giant right in the middle of Jorgunfist and Xin Shalast (Mokmurian's two bases of power).
So, as I contemplate how to run a Giantslayer AP one day (certainly not until all 6 volumes are in my hands), what are some ideas on how I could handle the two Giant Lords in close proximity to each other?
If Mokmurian were alive in your game, how would he react to Volstus?
How would Volstus react to Mokmurian?
How could I justify leaving Mokmurian out of a Giantslayer campaign?
How would I include him?

I'm not sure, but from what I read from the current Giantslayer volumes, Volstug has an orb of red dragonkind, aka he can dominate red dragons. That alone would make him more powerful than Mokmurian.
If you want to include Mokmurian, you could either have him as one of the giants who swore fealty to the Storm Tyrant (after all, he did swore fealty to Karzoug). Mokmurian would make a nice vice-BBEG, as Volstug's second in command and adviser.
Alternatively, you could have Mokmurian be a rival to Volstug, trying to wrestle control of his army for himself in an 'evil versus evil' situation. In that case, he could make a valuable (if treacherous) ally of the PC.
If you don't want to include him, simply say in the background his stone giants have refused to join Volstug, much to the latter fury's.
Just suggestions of course.

I love it.
He could be the infamous "Death Adder's Trainer" at the end of Golden Axe - the Giant behind the Giant. Beef him with some reasonable levels, and have him come back to smite the PCs after whatever the aftermath of Volstus' brief reign might be.

The power Volstus has achieved without interference from Rune Giants, that we know of anyway, is quite impressive in and of itself, as they consider at the very least what was once Thassilon their personal stomping ground.
I'd say it's possible for Mokmurian to be entirely independent of Volstus. Hell, after that stone giant inquisitor of Minderhal leaves Volstus' own forces, he could ally with Mokmurian instead, and it could be a post campaign type of situation with a resurgent Minderhal faith and a unified kingdom of Stone Giants.
Giant kingdoms, while having a vast range, don't strike me as all too populated with Giants themselves. A big city might have 300 MAX or so. Who knows.
He could be someone who wants to see Volstus fail, so the old enemy of my enemy bit could definitely apply here. And he could be helping things behind the scenes even.
Maybe he could be working with designs to actually get the orb itself away from Volstus.
