I can never remember the order of a champion's codes. My 1st printing CRB is too errataed to be trusted, and Archive of Neyths spreads the information across too many pages.
Are there any character generation tools that will lay out the full code when you create a champion, including the proper order of the anathema?
With exception of Remaster Compatibility errata there's no changes in the champions anathemas since the 1st print. So if you are playing a pre-remaster game you can use it normally.

Once per cemented character concept, sure. But its also something that will vary if I can't settle on a god. It also is just more confusing than it needs to be. See this thread:

Hopefully PF2 simplifies this.
Incidentally, a friend pointed out PF2.easytool gives you a comprehensive code for the old version. Not sure if they will institute the remaster CRB eratta or just wait for Player Core 2. Ex:
