OK, Ex-Asmodean reads this way: "Choose one: you gain a +1 trait bonus on attack rolls and weapon damage rolls or you gain a +1 trait bonus on the save DCs of your spells against agents of House Thrune and worshipers of Asmodeus, including most (but not all) devils."
How widely do I apply this for my PCs? Do I give that to a PC when they fight the Chelish Citizens thugs? What about Dottari? How do I know who worships Asmodeus?

You could read "worshipers of Asmodeus" like "members of the church of Asmodeus" in this case. To interpret the trait like this will ease playing this out in game. Otherwise you have do decide for each NPC individually, I guess. And this will be nasty work and just confuse you and your PCs.
I would do it this way: The trait applies for agents of House Thrune, members of the Asmodean church and devils that are underlings of Asmodeus (so almost all devils). Chelish Citizen thugs likely worship Asmodeus, as well, as they welcome Chelish rule... so I would include them. Let your players apply the bonus to important/named NPCs that you think or you decide to worship Asmodeus but not to every guy they meet in the streets. Most of the Kintargan citizens just pay lip-service to Asmodeus but worship other gods in private so don't waste thoughts on random NPCs. Since most of the Dottari guards are "normal" people, most of them don't worship Asmodeus as well... but I would apply the bonus to high-ranking Dottari officers since Barzillai made Vanesses Trex commander of the Dottari and he or Trex likely replaced other officers in charge.

Focus on the agents of House Thrune part; the worshippers of Asmodeus are likely the clerics and inquisitors. They're mostly agents or worshippers if they're fighting you, unless they're cat's paws (Queensmen), Hellknights (an NGO), animals and monsters in side quests, random encounters or the like. And I could see the argument for the Queensmen being agents.
