It was possible for some folks to miss the checkout step for the physical book, which required folks to scroll down a bit on the page to find a mini checkout section. I imagine this is what happened in the case of your Humble Bundle redemption, so while your book was redeemed, it was sitting in an unsubmitted state. I've gone ahead and fixed this for you and you should see a confirmation email shortly! As mentioned above, you can expect your book to ship out in November due to the backorder status of the book.
