Karzoug has an adult blue dragon with him, but there's also a very old blue dragon named Ghlorofaex serving him. However, Ghlorofaex isn't normally encountered together with Karzoug.
It may also be Kazavon, who's probably the most famous blue dragon in history, but Kazavon had no relation to Karzoug.

Originally, we just put a blue dragon on the cover because dragons help sell products. Simple as that.
Then, when I went and revised the Rise of the Runelords campaign for its hardcover version, I put a blue dragon in that final encounter with Karzoug, as an hommage to the cover for the GameMastery Guide.
THEN, when we ordered the cover for the 2nd edition Gamemastery Guide, we put another runelord and another dragon on the cover as a callback to the 1st edition cover. But as in the first case... it's not any one dragon in particular. It's just there to make the cover look cool.
