So, at my table, not only did Octavio come to the Ruby Masquerade, but he died there. Then the party rolled a one on reading a raise dead scroll above their level. I made it so that Octavio acted seemingly like his normal emotionless self. So, what's the twist? Something to do with the Soul Anchor? Something related to Barzillai Thrune? Something else?

Probably too late for this, but:
Reincarnation as a previous NPC is really good. The more I think about it, bringing him back as an antagonist fella punitive, I'd go for someone interesting like Brakisi or Rexus.
That said, the temple hill slasher would be delicious, even if he's a demon by now. Maybe a fragment of his soul?
Barzillai's sister is canonically alive, but don't let that stop you.

If Octavio becomes Mangvhune, all the questions in book five about how he knows anything about the current Silver Ravens get solved pretty well.
If back as Barzillai's sister, you potentially get some great melodrama as she appeals to her evil twisted brother to stop the madness and whatever you think Barzillai's twisted response to that would be.
Nox, after the Broken Soul incident, could be a potential ally.

To be fair though, just having spell fail and giving them another chance to resurrect them would be nicer to players especially since by default there isn't that kind of raise dead fumbles in 1e :p
I think having him become another character straight up is kinda too confusing and creates too many variables to take in account
