Mogmurch is at a location without any combat option in his hand.
Alahazra recharges a blessing to examine the top 2 cards of Mogmurch's location (this is 0-A & Alahazra doesn't have the ability to re-order, BTW).
Card 1 is a monster with a Trigger that forces Alahazra to encounter the monster. Alahazra also has no combat options & buries a Camel to evade the monster.
Card 2 is a weapon.
We need to shuffle the monster back into the deck because Alahazra evaded it.
But does the weapon shuffle in as well, or stay on top of the deck?
We ruled it stayed on top of the deck.
Mogmurch discarded an ally to explore, and encountered the weapon.

It was already discussed in one of the more extensive threads on examine/Triggers, but the key is this sentence that's lacking from the printed Rulebook:
"If anything would cause you to shuffle the deck while you
are examining cards, shuffle the deck only after you put the cards
back."
So, if only a single examined card causes you to shuffle, the whole "put the cards back in the same order" goes out the window - you shuffle any and all cards in the location deck (except for the "displayed on top of deck" cards)
EDIT: (Frankly, this addendum to the Rulebook solves so many questions and edge-cases about all the possible examine/Trigger interactions, that I think it would be nice to get a specific FAQ on the site, in favor of all those players that don't keep up to date with their downloadable Rulebooks; to my knowledge, they couldn't even know there was an extra crucial rule added, as the updated MM Rulebook was the very first version posted for download, so there was no way to guess it was different than the printed one)

So, (vaguely) related question: what happens if you get moved during a sequence of examines? For instance, if I use a Blessing of Nethys to explore a location, and the first examine hits something that moves me (I don't have the cards in front of me, but this came up with one of the early adventures - Sandstorm, maybe?). In particular:
a) Does my second examine hit my new location or my old location?
b) Does the attached explore hit the new location or the old location?

a) Old location. If you're told to examine one or more cards, they all come from the same deck. The power is worded "Examine the top 2 cards of your location deck" not "Examine the top card of your location deck, then examine the second-from-the-top card of your location deck"
b) New location. You always explore your current location. MM rulebook p7: "When you explore, flip over the top card of your current location deck."
