ME (human with club) VS BROWN BEAR
Mr. Bear successfully grabbed me with his claw attack and now I'm grappled (bear won initiative).
On my turn, I want to hit the bear with my club instead of trying to break the grapple. So I need to roll a grapple check to beat his CMD and then make an attack roll with my light weapon? Or do I just make the attack roll without the need of the grapple check?
Now it the bear's turn, we're still grappled, so the bear needs a successful grapple check with +5 bonus to deal claw or bite damage without needing the additional attack roll for the attack?

Instead of attempting to break or reverse the grapple, you can take any action that doesn’t require two hands to perform, such as cast a spell or make an attack or full attack with a light or one-handed weapon against any creature within your reach, including the creature that is grappling you.
A grappled creature takes a –2 penalty on all attack rolls and combat maneuver checks, except those made to grapple or escape a grapple. In addition, grappled creatures can take no action that requires two hands to perform.
So you can jsut use the club to attack with a -2 penalty, no CMB check needed.
On his turn, the bear can choose to maintain the grapple with a +5 bonus. If he maintains, he can automatically do damage equal to one of his natural attacks.

You seem to have a fairly solid handle on this.
You don't need to make a grapple check to attack with a light/one-handed weapon.
The bear does not need to make any additional attacks it just makes a grapple check with +5. If successful it has other options, not just bite or claw damage. It can move you up to half it's speed, taking you with it, or it can pin you (technically it also has the option to tie you up, but that seems unlikely to happen).
