Weapon / Armor enhancement increase?

One thing that isn't clear to me about enhancement bonuses for weapons or armor is whether or not you can increase them. Is it possible to do so? If so how exactly does it work?

yes you can. You work out the final price of item and subtract the current price of the item.

So if you have a +2 (8,000gp)sword and want to make it +3 (18,000gp),
you enchant it as an item priced at 10,000gp.

It's not doable by anybody, you need someone who can craft magic weapons and armour (probably via the 'Craft Magic Arms and Armour' feat).
They can increase the bonus just as if they where adding new abilities, as per these rules.
