I'm wanting to build a TN Cleric of Pharasma (or Nethys, not sure which yet) that will prestige into the Envoy of Balance prestige class in Champions of Faith and was looking at the endowments they gain. Spiritual Equilibrium is the first one I'm getting, but I have questions about the Twinned Channeling endowment. How exactly does it work? Does it both heal and harm the same characters at the same time? Or do you choose which character gets healed and which gets harmed?

Yes, both the heal and harm effects occurs simultaneously to everyone in range.
If you have Selective Channel, you may select different (or the same, if you wanted) creatures to NOT be healed and to NOT be harmed.
Let's say that I have Charisma bonus of +2. I could spare two living allies the harm and spare 2 living enemies the healing.
Note: You cannot heal twice (through use of race/class features or feats) because you only do heal with one and harm with the other (unlike the reversing effects on Cure/Inflict spells).
Note: You need Spiritual Attune ent endowment first.

Selective Channel can be applied twice-- once for the healing wave and once for the harming wave. The people selected out of each wave can be the same or different, although I'm not sure why you'd remove one person from both (some situation might benefit from it, I guess).
Joe the Envoy, Bob, and Pete encounter goblins Un, Dos, Tres. Bob and Pete are a little banged up. Joe has a Cha of 14 and Selective Channel. He channels, selecting out himself (inherent), Bob and Pete from the harm and selecting out Un and Dos from the heal.
Joe, Pete, and goblin Tres heal.
Un, Dos, and Tres are harmed.
