When my group completes the 0A scenarios (either 0-0A or 1-0A), do they get to do a normal deck upgrade as well as the scenario reward? I assumed that the deck upgrade would occur as normal so that, at the end of the scenario, my character deck would potentially have two different cards than when I started - one from the scenario reward and one of the boons that the party gained during the scenario (suitably turned into a card from my class deck).

The scenario reward is a single deck upgrade. You don't play your normal characters during the X-0 scenarios, and you may not have even gotten one yet. Even though the instructions do say you fill in the character's name and class, if they don't have one I generally just let them decide on that when they actually apply the mini-chronicle.
Also note that you can only get the reward once for the X-0 scenarios - it's not per character.
The Wrath demo/intro scenario has been revised to give out the mini-Chronicle sheet as the reward, so hopefully that will clear up one axis of confusion. (If somehow the party fails the scenario, give out the mini-Chronicle anyway.)
And yes, the mini-Chronicle sheets are 1/player, not 1/character.
