75%/50%/25%. -1/-2/-3; applies to caster level as well as the other stuff (which means it can lock a caster out of his highest-level spells temporarily). A GM who wants a "gritty" campaign can double the penalties.
Under the system, Endurance reduces the penalties by 1. A couple of new feats as well.

Is it designed to apply to the enemies as well? It seems that it should to be balanced, but keeping track of what percent all the enemies are at, the corresponding penalty and all that for multiple bad guys in a combat seems rough on the poor GM.
They address this in the book. Mass groups you track above 50% and under 50%. They take a -2 below 50%. Named (iconic) NPC's take the full wounds system.

I would not recommend it for dungeon crawling games, but I feel it would add nice tension to urban crawls. Ambushes get lot of priority when sheer damage can greatly debilitate your opponents.
I got this neat feeling that the system would make perfect mix if used with Unearthed Arcana's reserve points. You get a chance to reset the spiral if needed.
