custom magic (or even mundane) items

I really like the feel of you don't just get a magic ring off every highwayman and/or tough enemy. So I was hoping to see some cool ideas for magic items! It would be especially great to see something completely unique.
For example I made a belt that pierces the abdomen and shortly after the pc won't feel pain. it has upsides and downsides to it but things like that are particularly interesting

The players are actually low level at the moment but I would still like to see magic items of any level.

And one more thing just to let you all in on what's going on there's a mirror the pc's have that lets them go into a pocket dimension once a day with a magic shop full of crazy stuff. The prices for things are in souls, rare materials, spells ETC other magical stuff so you couldn't just buy it from anywhere! The owner of the shop is an eccentric demigod


So, the general problem with this is that normal pathfinder assumes basic access to certain items to help characetrs combat CR appropriate encounters. What you are describing is a low magic world, which is not the normal operating paramters for pathfinder. It's fine to play this kind of game, but you should be aware of the limitations.

I agree that it would be nice to have magical items be more unique and avoid the christmas tree effect that is required to be a good combatant, but that is how the game is set up. By removing access to magical items you hurt martial characters even more (who are already weaker) because they cannot buff themselves as others can.

Firstly I would recommend you look into giving your characters some sort of free progression that basically grants the bonuses of the big six for free.
1) Magic Armor
2) Magic Weapon
3) Amulet of Natural Armor
4) Ring of Protection
5) Cloak of Resistance
6) Headband or belt of stat increasing

Outside of that, you don't need to make super custom items. Just go through the existing items and pick out interesting wonderous items, rings, etc and give them to players as loot. Leave clues for players to find out what the past of those items are (that you have generated). This will make them unique and treasured, and the players don't need to worry about not having certain bonuses because it will be baked into their character progression.
