Hello! I'm looking for a convenience way to have some printable form of cards for summons. I don't want to go through the hassle of writing down all the stats and I like to avoid using electronic devices as it takes up too much time and is harder to take notes. I'm looking for some sort of premade stat block formatted in a way that can be printed (4 or so per page?) or a blank card of some sort that I can print and fill out by hand. I saw a link to one as pathfinderdb, but the link is broken.

If you want a blank card that you can fill out by hand, I'd just go the simple route: Buy cue cards, then fill out each with your summons as you go. Copy from the book and add modifications as necessary from feats and such.
I list the stats, attacks, hp, saves, AC, etc... on the front, then list the skills on the back.
I do the same with my Cohort, use the standard sheet for leveling and showing the math and such, but putting all my "need to know in combat" information on a cue card. Once you find a layout that works for you, just rinse and repeat so you have a standard system for ease of reference

When I ran my first 3rd ed wizard, I used index cards, a hole punch and a keyring to keep all my spells straight. I've also used it for alternate forms for a polymorph. A similar concept for summoned monsters would probably work, though it's a lot of writing - especially if you get Augment Summoning later and have to re-stat based on that. But it'd be a huge time-saver at the table.

Nathanael, summon cards are relatively easy. There are only two major versions of any summoned creature with a few other possible variations (such as adding the young/advanced/giant templates to a couple versions of summoned creature if you are a shaman). This is probably why there are multiple published tools for summons.
However, Polymorph (Wild Shape) cards are not as easy. Each Polymorph (Wild Shape) form is significantly dependent upon your base form (HD, HP, BAB, Base Saves, etc) and upon what equipment you are wearing.
That is why I created the Polymorph excel sheet to automate updating the Polymorph (Wild Shape) forms.
