Though it's tough for any of us to talk about underplayed scenarios, because our perspectives are very limited, I'd say...
-Red Harvest (best scenario of Season 3)
-Eyes of the Ten I: Requiem of the Red Raven (best scenario ever)
-The Jester's Fraud (up there with Dalsine Affair in Season 2)
-Hall of Drunken Heroes (Season 1, pretty solid if The Weird Part is cut out)

Yep, Jester's Fraud is so underappreciated that I messed up its season. Turns out Jester's Fraud is Season 1.
Delirium's Tangle is good, too, but it supports low levels, meaning that it likely doesn't have the "underplayed" problem.

Since you specified "which very few people have played" I'm going to have to answer the question with
"The whole Eyes of the Ten series"
Especially Part 3. I know it is often cited as the weakest of the four but in the right circumstances (when you don't have a time limit and have a good role-playing group) it can be an absolute blast. If the GM really plays up the environment you are in and the reactions of NPCs there is pretty much limitless potential.
But the whole series is absolutely amazing.

Hall of Drunken Heroes is awesome (mostly BECAUSE of the weird part).
The look on their face when the ID that spell is so worth it.After that I truly enjoy
Rebel's Ransom (An honest to goodness dungeon crawl and FUN to build)
Sarkorian Prophecy (She's my favorite 2hd wielder ever)
Pallid Plague (A scenario that encourages skills AND improv? Woot!!)

I will 3rd Hall of Drunken Heroes, especially when the Weird bit is played to the max. My group when I played it, were able to decimate it, (helped that we all made our saves, somehow,) but we still felt the pressure. When I ran it, the player reaction was awesome. They got decimated, but they reacted well, and when they found out they weren't really dead, they were amazingly relieved. Didn't hurt that I described their horrible, bloody deaths in minute detail. :P

I enjoyed GMing "Our Lady of Silver", mostly because of its interesting roleplaying opportunities with very high-ranking NPCs. I also enjoyed running the combat against the
