It does not change type. It changes what abilities you have while you are that type by copying another type's abilities. If it changed type it would say so. The rules are generally more inclusive than exclusive meaning there is no reason to say "this doesn't change your type." If you were a humanoid before you're still treated as a humanoid during a polymorph effect for things like spells.

I'd argue that your class abilities are almost never dependent on your form. They are dependent on your class.
An awakened bear would still possess the amphibious quality from his class, so why not a human druid wildshaped into a bear? Ergo, it is not dependent on form.
Certain racial abilities, such as grab or darkvision, on the other hand...

because an awakend bear would add amphibious to his beary self. If you use magic to turn yourself into a bear, you don't use it to turn yourself into an awakend bear that has levels in druid :P and considering you can gain those types of abilities with changing your shape, using your argument, you couldn't gain those abilities because you magically take a different form, yet they're not depended on form right?

You would not lose the subtype. You might lose abilities the subtype grants to you however. Polymorph does not affect type or subtype.
As an example if a subtype of a humanoid granted flight due to having wings and you changed into something without wings you are still that subtype, but you can't fly because your wings are no longer there.
