I apologize in advance for this question as it is very basic, and i didn't see the specific question in the forums. So, here goes:
1. Do you provoke an attack of opportunity by moving INTO a threatened square? In other words, at the beginning of every fight, as soon as my character runs up to a monster (or vice versa), does the monster get an aoo? To my eyes, the book seems to contradict itself, saying in one place that moving AWAY from a threatened square will provoke an aoo. But, then, the chart indicates that any move action will also do it.
1.a. Moving from a threatened square to another square of the same monster: does this provoke an aoo? (I thought of this while i was typing, lol).

1. No; but if the monster has reach 10 feet, you DONT provoke when you get to 10 feet of it, but you DO provoke when you move to be adjacent to it.
1.a. If you take a single 5 foot step, no. Any further than 5 feet yes.

To clarify your confusion, the chart doesn't say all movement always provokes, nor does it say that all movement MUST provoke. It merely says that movement might provoke based on the full rules about Attacks of Opportunity. So the "yes" in the column really just says "Yes, this move might provoke if it meets the other conditions of AoOs."

1. No you do not provoke an AoO by moving Into a threatened square, unless you move more than one space through the opposing force's threatened area.
Moving through, or out of a creature's threatening area, will provoke an AoO. The chart is a little funky and not that fantastic to rely on. Always read the fine print when it comes to Pathfinder.1.a. Yes it does, and only ONCE for moving through a threatened area.
There are feats, abilities, and skills that can potentially change this. For instance, having more than 3 ranks of Acrobatics trained will allow you to do an acrobatics check to avoid an AoO when moving out or through a threatened area.
