An ettin normally gets a full attack with each arm, as it's controlled by a separate mind (and have the Superior 2 Weapon Fighting), so I wouldn't see any reason they couldn't choose one arm to fight with a different weapon.
Normally an ettin gets its iterative attacks with each arm separately if it's full attacking.

Basically for the purposes of attacking treat each arm as a different person. Basically he could cook stir fry with one hand and attack with the other at no penalties. Although you would be able to identify which head is the chef by his hat.
So can an Ettin get 2 ranged AND 2 Melee attacks in one round?

Well assuming the Ettin is holding a javelin in one hand and a flail in his other hand. He could then melee attack using his flail against an opponent with one hand, and hurl his javelin with the other without penalty.
He'd only get to have one ranged attack though, since he lacks a means of getting another javelin in his javelin hurlin' arm in the same round.
If he had picked up the Quick Draw feat and had spare javelins, I believe he could have 2 Melee and 2 ranged attacks as you said.
