Can someone clarify for me:

So if I am a level 4 fighter with a +4 BaB and I transform into a creature, like a crocodile, with 2 attacks (bite and tail slap) do I get both attacks or just one since my BaB is for 1 attack?

Both attacks, but you would not gain an iterative attack at 6 BAB.

A tail slap is a secondary attack, so you would have a -5 to hit with the tail slap, compared to the bite.

Marie is quite right. To expand on what she said so that you can understand for future rules questions you need to look at the natural attack rules. They function a bit different from manufactured weapon attacks.

If said fighter turned into a four-armed monster with four claw attacks, he could attack with all four claw attacks without penalty (because they are all primary attacks) with a full attack action.

He could also wield a two-handed weapon in two of his hands, making his normal attacks with it (including iterative attacks, if any), and then also strike with his two remaining claw attacks at -5 to hit (because ALL natural attacks become secondary when used in conjunction with manufactured weapons).

This is super helpful! Is there any errata to cover abilities not listed in beast shape? Specifically I am thinking of the weasel and its attach ability.
