Apollyon is the horseman of pestilence.
My party is going to explore an ancient, abandoned temple too Apollyon. The only Apollyon specific item I could find is a level 20 ring.
Does anyone have suggestions for some cool-flavorful items they could find there that would be appropriate for a 5th level party?

How about a tattered Cloak of Resistance +2 or +3, with the ability to cast Absorb Toxicity 1/day or somesuch? It's a mid-level spell, but has very fringe usage.
Alternatively it could cast Contagion, but that'll probably go unused by the PCs if they aren't Evil.

That's kind of a tough question without knowing more about your PCs. (I usually like to somewhat tailor treasure to my group.)
Since Apollyon is the Lord of Flies and Pestilence, just some ideas off the top of my head -
The Plaguebringer's Mask from the Curse of the Crimson Throne AP might be cool if you want minor treasure. Maybe the mask looks like a fly's head when you put it on? You could add the effects of some magical goggles (minute seeing?) to it, to mimic a fly's eyes.
Apollyon has a sickly yellow scythe that he uses, so a magic scythe is always a possibility. Probably only a good option if you have a scythe user in the group. Maybe the scythe inflicts mummy rot on a successful critical hit or something.
An ebony fly - figurine of wondrous power? You could make the fly sentient, able to talk in Abyssal, etc. Maybe the fly is really a bound quasit loyal to Apollyon. (Instead of being able to turn into a bat, the quasit can turn into a large fly as an alternate form.) Could be a magic item or one of the PCs could take the quasit as an improved familiar.
A magic crown that allows the wearer to case summon swarm 3/day (flies or locusts).
A wand of summon swarm, or a talisman that gives a bonus to summoning spells that call flies or diseased creatures.
