That's the chronicle sheet for the players. I'm looking for the GM Sheet that gets passed around at the start of the game and all the players put their name, ID number, and faction on it.

AFAIK, there isn't one. Just use a blank piece of paper. Ask for character name, PFS #, and faction.

Who ever will be reporting the event can download one specific to that event and give it to you.

Yeah, those "GM sheets" are purely for convenience only. You can have people sign in on an index card or something. The Fox noted all that needs to be included

Generally, it is the responsibility of the person reporting the event to provide you (as a GM for the event) with a tracking sheet. They then collect the sheet after the game and report the table on Paizo.com

If you are the person responsible for reporting the event, you can download a tracking sheet from your My Pathfinder Society section on Paizo.com. Under the GM/Event Coordinator tab where you create the event there is a link to "Download session sheets". This will give you a PDF with 3 tracking "sheets" on one page.
