This horrific amulet forged from the chains of kytons digs into the flesh of whoever wears it and refuses to let go. If damaged, the wearer suffers that damage instead; the pain nexus cannot be destroyed while attached to a living wearer.

Whenever a creature within 30 ft. of the wearer suffers hit point damage, that creature takes only half damage and the wearer of the pain nexus takes the rest, as shield other . This does not stack with shield other.

The pain nexus may only be removed by effects that remove curses or if the wearer dies.


That's horrific for any melee character since it doesn't differentiate between friend and foe. Definitely a good cured item.


I thought about adding a rule where if it's unattended, it attempts to grapple adjacent creatures and if successful, embeds itself in them.

But that might be too cruel.

Just make it look like something the players would want to wear and there's no need to make it grapple.

It's a good gimmick to explain the ravening critter that's not ravening. Purple worm, scraping fungus off walls, ignoring the gnome? Gelatinous Cube that avoids living things? :)


Evil humans might make it into a crown and try to send it to the king.
The party intercepts it, then it's hard times for the party member who always calls dibs or just grabs stuff.


It combines quite "nicely" with the Beguiling Gift spell.
Also: get one of the Familiars with Regeneration, make it wear it and the whole party only ever takes half HP damage. The Familiar can't die from the damage anyway.
