Hey guys, been a while for me since I've been on these forums! I was just wondering, I have a friend who wants to play an anti-paladin in a game that I'm intending on running in the next little while and he wants to know if he would be able to come into the game loaded with every disease in the pathfinder universe. Given that anti-paladins are immune to all disease effects and can still transfer them.

My questions are as follow: 1) is it technically possible for him to backstory all the diseases into his character and just have a new guy rolled from scratch having everything?

2)When he starts punching everything with metal gauntlets and stuff (effectively dealing lethal damage) will the disease he has be contracted by the enemies?

3) do enemies take the penalties/have to roll the saves for the diseases immediately, or do they have to wait the intended 1-6 days (given the disease and the roll)?

4) In reference to question 3, are there specific disease that have an instantaneous effect on enemies that require them to make saves immediately?

5) Is this even a real thing? I was looking over anti-paladin and I guess it makes sense given their Plague Bringer trait, but can you have every disease and transmit every disease when you punch things? I really don't mind, but the idea of having recurring enemy NPCs fighting against the party and having to make 20+ disease saves for them after the fight (if they escape even :<) seems poopy.

It sounds a bit silly to have every disease. At some point the diseases start competing against each other I imagine.

Almost all diseases work slowly, at a speed of days. So they're not very valuable in combat. Exceptions probably exist. Tell your player that you refuse to look through every book for them. He can do the research, then you'll take a look at the stuff he's researched and decide if you like that disease. Maybe make him do some skill rolls to see if he's managed to collect that particular one.

Recurring enemies... that's actually pretty hard. Players tend to be vicious and don't like to let anyone escape. Many players also never consider retreating themselves, even in the face of superior foes. Any enemy that can get away or force the characters to retreat is going to be powerful. If he's prepared to face these particular characters, he could have bought a Periapt of Health.

So it's an interesting flavor for an antipaladin, but it's not a really practical thing. Keep an eye on Mummy Rot and Lycantrophy though. Lycantrophy is normally only contagious on True (not infected) shifters. Mummy Rot is a particularly nasty disease/curse, but I'm not sure it can be spread to other people (by anyone other than mummies).

Also think about the social implications. This concept makes it really hard to interact with any organized society with a concept of quarantine. Also, the other PCs might be exposed to the diseases too.

Oh, and there's the usual "are you sure you want to run an evil campaign" stuff. This does sound like a somewhat immature character concept. Good for a laugh, not necessarily long-term campaign material. I do think many new players just need to give stuff like this a spin once to get it out of their system; people need/want to try some immature stuff before hunkering down to play saner characters.
