Total character level or, equivalently, total hit dice. At level 1/1, you don't gain a feat for your character level or for any racial ability that operates at first level, though you get everything that the first level of the new class provides, such as a bonus combat feat for being a level 1 fighter.

If a character multiclasses, do they gain feats based upon their total Hit Dice, or based upon each class level?
For instance, I gain a feat at first level. If, when I reach second level, I decide to take another class, do I gain a level then too because it is 1st level?

Yeah, it's based off of your total character level. If I were a 3rd level Human Cleric/Fighter, I would have a total of 4 feats; 2 from Levels (it's 1,3,5,7,9... for when you receive feats as starting with a basic class from 1-20, or as Starglim put it, total HD, which is the total level of your character), 1 for being Human, and 1 as a Fighter Bonus feat.
If I was Cleric for the first 2 levels, I would only have 2 feats. If I went Fighter that next level, I would gain 2 feats; one for being a total level of 3, and another one because of the Fighter's 1st level Bonus feat.
