This ability otherwise functions like and replaces the standard witch familiar.
Does that mean that the mask gets the same level dependant abilities as a normal familiar?
Like alertness, empathic link, improved evasion and share spells? And does that mean that the mask gets an intelligence score and thus becomes an intelligent item?

yes to all, except intelligent item. It is an item, and it is intelligent, but it does not have any abilities from an intelligent item.
Just think of it as a paralysed monkey familiar wrapped around your face, that you can later enchant as an item. I do not know how it would be handled if you would make it an intelligent item of its own. If you're interested in that I would happy to flag your post for FAQ.

You mean it would be an item with an intelligence score but does not have any special abilities normal for intelligent items?
What I really wanted to know was:
- Do I get Alertness when it is near me?
- Can I cast personal spells on it even if the spell would not normally work on it's type? (via share spell)
- Can it roll knowledge and/or perception skill checks seperately from me.

Hello I have been looking up things on the Orc witch doctor and I keep seeing these posting about the mask. The text reads:
Her fetish mask acts in all ways like a witch's familiar for the purpose of preparing and gaining spells.
from reading that I am pretty sure you do not gain any of the abilities a familiar gives you like alertness or so on. It says pretty clearly "for the purpose of preparing and gaining spells" The other abilities granted by your familiar are not bestowed, but you go get the +2 intimidate and heal checks when wearing it and that, I believe, is the trade off.
