Yes, Concentration is required. See the grapple condition on CRB p567.
In addition, grappled creatures can take no action that requires two hands to perform. A grappled character who attempts to cast a spell or use a spell-like ability must make a concentration check (DC 10 + grappler’s CMB + spell level, see page 206), or lose the spell.

Grapple is still a difficult combat technique to run as a GM but here goes:
Round 1 Grappler: Make attack roll, if success -> Make CMB check with a +4 bonus for grab (usually represented in the creature's stats), if success -> Apply Constrict damage (if any).
Round 1 Grapple victim: Make Escape Artist or CMB check and compare to Grappler's CMD. Alternately, make attack rolls instead of escape rolls to do damage instead.
Round 2 Grappler: Make CMB check (+4 for grab, +5 for continuing grapple), if successful apply damage from limb making grapple check, apply constrict damage (if any), make rake attacks (if any).
Round 1 Grappler: Make CMB check with a +2 bonus for Improved Grapple. If successful you are grappling.
Round 1 Grapple victim: Make Escape Artist or CMB check and compare to Grapplers CMD. Alternately, make attack rolls instead of escape rolls to do damage instead.
Round 2 Grappler: Make CMB check with a +7 bonus (+2 for improved grapple, +5 for continuing the grapple). If successful apply damage from an attack.
