I've been looking for an answer for this for a week now, but I have yet to find an adequately helpful person or existing Internet thread.
I've run into a problem (one I've never run into before) where, first, I couldn't REMOVE a character (Khelgar, to be specific) from my party - his name was faded out in the party roster, just like when you're supposed to be required to take a character for certain quests, but this was not during any of those quests. After that, I couldn't ADD him - he'd disappeared from the party roster entirely (though not from the inn, he was still there and I could still talk to him). How can I fix this? I have two other games (also the primary campaign) with two other characters, and neither of them has this problem.
I've seen a lot of extant threads talking about some well-known bug where magicians' familiars replaced characters when summoned and you transferred to another area - this is not that bug (I don't think).
Anybody know what I'm talking about? Any help with this? I should already have all the patches, and I also have several of Kaedrin's PrC Pack patches (though no others, just the official patches and Kaedrin's).
Please excuse my clumsy writing, I hope I'm making myself sufficiently clear.

Thank you very much, but apparently, that's not enough by itself.
There's the little problem of me being unable to open this ROSTER.rst. folder into anything legible. The threads you sent me to talk about "LETO" and "GFFEditor", etc. - what are these, do I need them? Do I really need to download some other program just to make this work? Where can I get them?

Honestly, in my days as an active NWN2 player, I don't recall any bug like this popping up in the game discussion (despite many bugs popping up and being discussed). I am wondering if one of the fan-made patches you installed actually created the problem. It may not be the case. But you might contact Kaedrin via the Vault if he is still active on it and ask him (and as he is a modder and has some knowhow, he might also have advice for a fix otherwise, even if his patch isn't the issue).
Otherwise, if in doubt, reload from a save before the bug happened. I know this is not the answer you want to hear, and it is a pain in the rear end, but sometimes that's the only "fix" to game issues like this. And if you don't have an old enough save, that's a lesson in itself--NWN2 is definitely one of those games that is "save early, save often"--and keep a lot of old saves.
If you have any further questions, I'd bug people at the NWN2 discussion boards; they are far more likely to be able to offer useful assistance, I'm afraid. Good luck.

I know this is an older post, but it checks out.
I got NWN2 via GOG earlier this year and had this EXACT SAME BUG with Khelgar.
I figured out a solution, and am posting it here in case it helps somebody else who runs into this bug.
(1) Go into debug mode (directions here: https://gamefaqs.gamespot.com/pc/922154-neverwinter-nights-2/cheats)
(2) Enter "rs kr_roster_edit" while in debug mode. That should bring up the "NWN2 Companion Roster Editor."
(3) Exit debug mode (press ~ again).
(4) Select option 3 ("Modify an individual companion").
You'll now see a list of all your companions.
(5) Click on Khelgar's entry (or the entry of whichever companion happens to be giving you this problem).
(6) Click on "Set companion as Selectable." For me, this bug caused him to be non-selectable.
(7) Click "Nevermind" twice, then "Nothing," and you should be good to go!
