Lets say an annoyed demon or Urgathoa is the one bringing this lycanthrope back, so we don't get into the logistics of how a PC would make this happen.
None of the template requirements are broken. When Lycanthropy is added it is a living creature. When Undead template is applied it is a corporeal creature. It is unusual. My friend might play a were-rat/zombie-lord in our upcoming campaign.
