I had a member of my group try this a couple of weeks ago. I've never done the exploding dice thing before. I wasn't about to then. Seems too powergamerish to me.

I've always been fond of 20/20/confirm, on both sides of the table. Adventuring is supposed to be lethal, or else every country bumpkin (no offense to country bumpkins) would be running around risking their lives. Combat is dangerous, and I've never liked being able to completely "math out" a system to the point that you know how much you can take and when to run away. Sometimes you don't get the chance to realize you're in over your head. Plus, with Action Point it's possible to negate such a roll on occasion.
So far I've only seen a 20/20/confirm hit twice. The first was a player killing an Umberhulk in one shot with a crossbow to the head on the first round. The second was a young Green Dragon ripping apart my brother as he charged in, also in the first round.

I've been in groups that have used the 3x20 = Instakill and the staging critical system (keep increasing the crit multiplier by one for every 20 rolled) and both can be extremely deadly. one game saw a fellow player killed by a peasent throwing a rock (instakill rules) and another saw me roll 4 20's in a row to outright slaughter the BBEG in one charge attack. both have merits, but the staging criticals system is the one that we stick with.

I remember this lucky fighter that used to get Trip 20's always during the BBEG. It was Poetic When i Trip 20 on him against a campestry.

"Yep the singing mushroom ripped your head off, Sorry."
