Here it is what happened.
I joined the Companions of Jorrvaskr and completed their quests, so i can ask one of them as a follower whenever i like.
I start the quest of Boethia, which requires you to sacrifice a follower who trusts you;i tried with a random NPC follower from the Dark Brotherhood, but he doesn't approach the column of the sacrifice (even thought he says he is going to do it).
So i have to choose another one, and i pick Ria from the Companions (the most boring and unuseful of them), and she works for the quest and is killed....only some time later she is back in Jorrvaskr, alive and kicking!!!!!!!!!!!!
I take her along on another quest and kill her again (i really didn't like her).
This time she doesn't come back, she isn't obviously following me, and no other follower in the game agrees to join me, they all say "you already have company" but i don't.

I think it's a bug that happens with characters that give quests or otherwise have parts in scripted quests. They have some form of coding that effectively makes them unkillable. Try killing the Thieves Guild, or a Jarl, and no matter how hard you hit them or what you use, they will never, ever, ever die.
I was playing on my Orc after finishing the Companions quest-line and oh hey that guy that got killed by the Silver Hand was back and offering me quests. What the ....?
My only recommendation is to go back to a save before you sacrificed her a second time and 'dismiss' her.

But if you eat him, you get Namira's Ring that lets you eat anyone for the rest of the game, significantly boosting your health and stamina. So what's a little cannibalism for a powergamer? ;-)

I just don't like the idea, and i know the rewards for any daedric quest (Prima games official Skyrim strategy guide); and my lvl 52 (for now) bosmer girl archer/thief is actually playing the front line tank herself thanks to the newly crafted set of dragon scale armor, all pieces double enchanted with a damage output about 70 (legendary glass longsword) or 105 (legendary glass bow) and she has to grow in power much more yet......so i think i'm powergaming enough even without the ring on Namira ;)
edit:forgot; armor rating around 400-410 and counting, not bad for a light armor

Skjor dies as a scripted event, so he died for me too, but he stayed dead everafter.Not that i really cannot play without a follower, playing a thief/assassin/archer my bosmer works better by herself.
On a sidenote: anyone else finds the Markarth quest No one escapes Cidhna mine ridiculous? It took me five minutes to escape that mine prison, you get the impression that the forsworn inside there were just waiting for you to be trown with them to escape.
