I'm not sure i understand your question but i'll have a bash.
Your main hand is the weapon you designate, your iterative attacks with that weapon go normally.
Your off hand weapon is the other weapon, you only get off hand attacks from feats. Two weapon fighting, improved and greater give you an off hand attack at 0/-5/-10 respectively using your base attack bonus etc, these function like iterative attacks on your main hand using the same bonuses and penalties.
Basicly...
+1 bab with two weapon fighting = one main hand and one off hand attack, penatlies apply.
+6 bab with two weapon fighting = two main hand attacks and one off hand attack, penalties apply
+6 bab with two weapon fighting and improved = two main hand attacks and two off hand attacks, penalties apply
this continues with greater twf and more bab.

multiple attacks from BAB (@ BAB+6, +11, etc) are always the primary attack. Fighting with two weapons gives you an off-hand attack. The two weapon fighting feat makes this easier, as does using a light weapon in the off hand. Improved two weapon fighting and greater two weapon fighting give you extra off hand attacks as you qualify for and take those feats. Extra attacks from other sources (ie, haste) could be from either hand.

Your main hand attacks are the ones you get through normal BAB.
You can always attack with your offhand weapon, TWF reduces the penalties for doing so to a manageable level.
Improved TWF lets you make two attack with your offhand weapon, one at normal penalties, one at an additional -5.
Greater TWF lets you make three attacks with your offhand weapon, one regular, one at -5, one at -10.

Made simple (without counting Strength, item bonuses and the other things):
1) You have Two-Weapon Fighting and your off-hand weapon is light: you get -2 to both primary weapon and off-hand weapon.
2) Now, you advance in levels and BAB; let's say you have +17 BAB.
Normally, you have attacks at +17/+12/+7/+2; using two weapons, you have +15/+10/+5/+0 with the primary weapon, plus a +15 attack with the off-hand one.
3) Then you add Improved Two-Weapon Fighting: it grants you a second attack with the off-hand weapon exactly at the same bonus that you use for your second attack with the primary weapon (BAB - 5 - 2). So you have +15/+10/+5/+0 with the primary and +15/+10 with the off-hand.
4) Finally, you add Greater Two-Weapon Fighting: as above, except it's "third" instead of "second" and "-10" instead of "-5". You end up having +15/+10/+5/+0 with the primary and +15/+10/+5 with the off-hand.
