You could use some of the mechanics on Oracle curses, lame, clouded vision..etc

I would think those who are born mute would learn some kind of sign language as a bonus.
....which would definately fit with the oracle mechanics, as they progress to overcome/master their curses.
Maybe there's something in the critical hit deck?
Or check the variant rules from UC. There's a called shots system that has special effects in that vein for crits and so called debilitating blows.
