They would follow the rules already in place for upgrading items. You would pay the difference between the +2 and +4 version and would need the Fame to afford a new +4 version.

That would depend on the rule allowing them (which it currently is "they are not allowed"). If it said "treat as a belt of giant strength +2", then that would probably be upgradeable. If it said "treat as item from the D20 SRD", then no, since gauntlets were +2 only.

I think you're misinterpreting it. Mike is saying substitute the belt for the nonexistent gloves because they cost the same and are the closest item.
