Actually you can have multiple cities within a hex, even though it is to no statistical advantage. A city does no "fill" a hex. The hex once designated as a city hex can not serve as a farmland hex though. You can however, build roads on a city hex.
Keep in mind that a "city grid" is actually a district and multiple districts can all be part of one city.
