You may gain one extra attack when fighting with two weapons.
The Two Weapon Fighting feat reduces the penalties you take when doing so.
The Improved Two Weapon Fighting feat allows you a second extra attack.
The Greater Two Weapon Fighting feat allows you a third extra attack.
There is no feat allowing a fourth extra attack in Pathfinder to my knowledge.
Thus, a +1 BAB character gets 2 attacks.
A +6 BAB character gets 3 attacks, and 4 with ITWF.
A +11 BAB character gets 4 attacks, and 5 with ITWF, and 6 with GTWF.
A +16 BAB character gets 5 attacks, 6 with ITWF, and 7 with GTWF.

I think Rogier was talking about four in total, two with the on hand and two with the off. As you pointed out, this is not the way the rules are currently written.

I'm going to bump this question because I'd like some clarity too.
I just became an 8th level ninja and got my first iterative attack. How does that stack with Two Weapon Fighting?
BAB: +6/+1
Relevant Feats and Traits: Weapon Finesse, Serpent Runner- Two Weapon Trait, Two Weapon Fighting, Weapon Focus: Wakizashi
+1 Wakizashi in each hand
Dex: 22 (+6)

How much of that progression can I use on an attack of opportunity?

Apoc, you're using the Two-Weapon Fighting rules and a pair of wakizashi, right? Those are light weapons, so your TWF penalties are only -2 on each hand. You get the same math, but it's easier like this:
Main 1: +6BAB +6Dex -2TW penalty +1SR-TWTrait +1WakiFocus +1Weapon = +13
Offhand 1: +6BAB +6Dex -2TW penalty +1WakiFocus +1Weapon = +12
Main 2: +1BAB +6Dex -2TW penalty +1SR-TWTrait +1WakiFocus +1Weapon= +8
Haste and Ki Pool stack, as far as I know. So using both of them it now looks like this:
Main 1: +6BAB +6Dex -2TW penalty +1SR-TWTrait +1WakiFocus +1Weapon = +13
Main 2: +6BAB +6Dex -2TW penalty +1SR-TWTrait +1WakiFocus +1Weapon = +13

You cannot ever do more than one attack as a standard action. There are a few rare corner cases that allow it, but you don't seem to have any of them at this time. So, on a standard action, you only get:
Main 1: +6BAB +6Dex +1WakiFocus +1Weapon = +14
That's it.
After a full move, you cannot ever attack, unless your full move was a charge, in which case you make one attack at the end of the charge:
Main 1: +6BAB +6Dex +1WakiFocus +1Weapon +2charge = +16 (but don't forget that you are also -2 AC until your next turn)
Attack of opportunity is always a single attack, same as your standard action:
Main 1: +6BAB +6Dex +1WakiFocus +1Weapon = +14
