I got the idea after watching a samurai film, where the hero used the reflection in his sword to spot enemies comming from behind. And I thought that this cliché is used quite often in films and such... so it should be in Pathfinder ^^

Blade Mirror:
As long as you wield a weapon with a flat, reflective surface you can use it to spot enemies behind you.
Game effect: You cannot get flanked.
Alterntive/Additional Effects: Bonus to will saves to see through illusions. Bonus against gaze attacks (since you view enemy through reflection and not directly).

What do you think would be good/necessary prereq's for this?
I also thought a shapeshifter bane weapon, whose reflection always shows the truth about a creature (basically true seeing... maybe slightly weaker) would be a cool item ;3

Hm.. yes. Just like this it would probably too strong. Only a few classes get uncanny dodge and it still can get circumvented.
I like the idea of only letting it apply to weapons with focus, but the prereqs are piling up quickly since I also like the idea of using up a move action (you do have to move the blade in position to look after all).
So you have to sacrifice offense for defense (as if fighting defensely, which basically gives the same amount of bonus you negate as penalty here. The difference is you can't get sneak attacked and you can only make 1 attack, albeit without penalty)
Blade Mirror:
Prereq: BAB +1, Wis 13
If the character wields a clean, bladed weapon made from a reflective material, he can use a move action to bring it into a postion that allows him to see what happens behind his back.
Doing this denies enemies the advantages of flanking. The effect lasts till the start of the next turn.
Greater Blade Mirror:
Prereq: BAB +6, Blade Mirror
Through extensive training the character can check his surroundings with but a single glance at his blade as it moves.
Checking his back now is a swift action.
Additionally when fighting an enemy while averting his eyes, the enemy does not gain concealment.
