I found the 2-headed template at the site: http://www.d20pfsrd.com/bestiary/monster-listings/templates/two-headed-cr-1 -or-2.

My question is how many attacks a 2-headed humanoid with a BAB of +6/+1 and a weapon in each hand would get when taking a full attack?

Would it be +6/+1 with both weapons, or +6 with each and +1 with a "primary"?

I tried searching the site and google and wasn't able to come up with an answer.

Unfortunately, this will be a two-part answer.

Rules-as-written: Three. The Improved Multiple Attacks feat states: "Two-headed creatures never take penalties to attack or damage from making multiple attacks." It does not, however, exempt them from the normal rules for making multiple attacks, which clearly state that when fighting with two weapons you make a single extra attack.

Rules-as-intended: Four. I believe the intent was to grant full iterative attacks to both weapons.

It also appears that the Ettin's Superior Two-Weapon Fighting ability has the same problem. Although the melee attack list clearly shows 2 flails each using +12/+7.

I think it would be much clear if they just said treat the creature as having two primary attacks (natural attacks, weapons, bites, etc.).


Thank you for the reply. Following that up... how would feats like Cleave work?

Using Cleave, if both hands hit, do they both get a second attack or would only the first one? Or does the wording on cleave "As a standard action... single attack", preclude this ever coming up?

I was thinking of creating a character with Improved trip and a flail in 1 hand, and something with a high crit range in the second hand, but the more I read, the more I wonder how well it would work.

It would be fun (possibly broken?) if I could Trip and then cleave with 1 hand, and then attack, and cleave with the second hand as well...

I've always wanted to run a monstrous campaign with an ettin controlled by two people. Each would function independently for attacks, skills and the like while movement would be dictated by whoever won an opposed charisma check. I think it could be fun role-play experience or a fitting punishment for two players that can't seem to work together.

Well, you definitely can't trip then cleave with one hand. As improved trip no longer grants you a free attack and cleave is a standard action in any case.

Yeah. Rules-as-written: a two-headed beast could either cleave, or use Improved Multiple Attacks to get two attacks, but not both because each is a standard action.

Rules-as-intended: I think you would get two cleave attempts for a standard action.

Compromise: Cleave still limits you to making one additional attack per round. So, I might say you get two attempts per round, but even if both hit you only got one extra attack instead of two. It does double your chances of get a successful hit.
