A fifth level monk is immune to disease, even magical ones. Lycanthropy is a curse so it would still affect the monk.
My PC thinks that he's immune. He could have gotten a cure but turned it down thinking he was safe. I rolled his save secretly and he failed. I know this will cause a problem when he changes. Just looking for backup on my interpretation and suggestions on how to handle the player.

Isn't lycanthropy a curse?
I'd give one of your knowledgeable characters the chance to catch that it is a curse and not a disease. This gives you an instant hook to try and find a means of removing the curse. Easiest would be to find some wolfsbane.
You could turn it into a quest for an item to remove the lycanthropy (the characters hear of a ring of wishes buried in some old ruins. Part of the challenge would be dealing with PC when he shifts. Do the other PCs construct some silver cage to hold him during the full moon.
Now, if the player is the type to try to play this as an advantage and not the curse, then you might have to do some more drastic things. Alignment changes would be in order thusly he wouldn't be able to continue taking levels in monk. The ultimate solution might have to be PC becomes an NPC and he rolls up a new character.

As far as your player, did you offer him a Knowledge check to know better?
Certainly players make their own decisions, but the characters are privy to the information available, and if something might occur tot he character, they should get a chance to roll it.
Lycanthropy is a bigger pain than mummy rot. It says it is a curse... but is treated like a disease for 3 days and is then untreatable, except by Remove Curse. For 3 days, a character attacked by a werewolf can be cured of lycanthropy as though it were a disease. But Monks are immune to diseases. But lycanthropy is a curse.
