Sorry if this is a bad place to propose a house rule, but in our games we do all our rolling in the open. I roll a d4 at the beginning of each round to decide if the dragon will breathe. It makes the combat a lot more exciting than rolling once and counting.
I think that is an awesome rule as it keeps the suspense of when the dragon is going to breathe up as you have no idea when he's going to breathe on you. Before you could guess especially when it had been 3-4 rounds and he still hadn't breathed on you you figured it was coming and knew to get behind cover.
