Just had an idea that i don't have the time to attempt so i thought i would throw it out here for others to consider.
How about converting Kingmakes to a large inland sea and doing it as a swashbucklery game with many islands? I've not read much about it in the (probably vain) hopes of one day playing it but the idea of an aquatic version of it appealed to me.
I started to do Kingmaker in a archipelago .You might want to change the location of K to put it between A and Z . My player was confuse why they carry loot farther to K instead of going strait to Z.

Black Bard and I have been discussing using Kingmaker rules on the Isle of Dread on the Savage Tide boards. Currently, I am in the idea formulation phase, but I believe Black Bard has already "hexed out" the Isle of Dread. We're always keen on receiving new ideas.
I'd look at Serpent Skull for Isle of dread ideas ;)

I had high hopes for the Serpent's Skull AP - that it would be more of a "Pirate Kingmaker" as you described... I was disappointed.
The Kingmaker AP is hard to follow - it's one of the best campaigns I've ever run (and I've been running games since the early 80's).
