His melee shows that he only gets one attack with the greataxe. Should he get a 2nd swing because he is a 6th level Barbarian?
He doesn't take it in favor of hitting with his horns and biting.
He probably should, but, uh... he's ridiculous. On the first round he crit and would've killed our cavalier if Pathfinder hadn't changed death by massive damage. The fact that he challenges one person to a duel is ridiculous. CR 10 vs. level 9. It's a bait to PK someone.
Our cavalier won, and he was a full BAB class with an AC of like 43. He still almost died.

Yes, but take into account that he has a hatred for gnomes, which might motivate him into rushing to kill a gnome on sight, and then take into account that the party can cheat and have more people fight him without concern of the redcaps joining the fight. Makes it fun. We just had this, and the fighter/hellknight almost made it through...although, she did almost get crit by his axe :P That would have nearly killed her.

So to clear up my confusion on this. Can he take a full round action and swing twice with the Great axe and then follow up with his natural attacks (Gore, Bite) Or is the second attack his natural weapons. Thanks!
He can swing twice with his axe
or
He can swing once with the axe and then natural attack
he is statted as doing the second one

Actually, the rules do support him getting both axe attacks. Take a look at the Horned Devil for an example. You can get full iterative attacks with your manufactured weapon and then any other natural attacks (that don't involve the hands you used). So if he just has Gore and a Bite then he should indeed get two axe attacks plus both of those.
Of course... yes, he is tough. Sacrificing one of those attacks isn't a bad option.
