I'm sure this has been covered in another thread, but my search-fu is weak.
In the Core book under CMB, it reads:
"Combat maneuvers are attack rolls, so you must roll for concealment and take any other penalties that would normally apply to an attack roll."
What are these "other penalties"?
Non-proficiency with weapon?
Penalty for two-weapon/multi-attacking?
Others?

yeah, pretty much. proficiency, moral, magic effects, curses, poisons, low ground, wearing armor, any other penalty that might be there.

Shaken among other conditions provide attack penalties. I think it would be quite time consuming to look up all the possible ways to get a penalty to an attack roll ;)

Just spend a slot to take the "Chuck Norris" feat - it prevents all combat penalties, regardless of situation, for the life of the character.

I would, but the prerequisite ("be Chuck Norris") is too much for my character.

Admittedly, it requires some character tweaking. :) First, you've gotta change your character name a bit - Falstaff "Chuck Norris" Deathdealer... then you gotta convince your DM to allow Comeliness as a stat (a la 1E - Unearthed Arcana) and then you have to make sure your score is LESS than 6... let's face it, Chuck looks like a saddlebag.
