Three times now, I get a survey when I log on to Paizo. It says Paizo Press at the top of the page, but says it has nothing to do with Paizo in the small print.
The Page is tagged "By Loud Mo".
The survey is about internet usage and offers a "free prize" for finishing it. Of course the prize has a Shipping & handling charge for more than the prize is worth.
I thought it might be a virus or spyware and ran a deep scan, but got nothing. I also searched "LoudMo" and it says it is "Pay for Instal Affiliate Program".
I went into my History, but the survey page isn't there. I did find the "Prize" page;

OK, I bounced from site to site and got it to happen again, this time at IO9.
This thing is originating from Direct CPV advertising Network.

Just like the one here, It "appears" to be a survey for the site you just logged on to, this one had IO9 on the title page, but in the small print said that it had nothing to do with IO9.
My pop-up killer is on and I have a few other add killers in my Firefox add-ons, but this thing slipped right by them.

Well after cleaning out my Cookie jar, it has not returned.
I had it pop up on two other sites, still pretending to be a survey for that site. The web address does not change and it is not a pop-up in the traditional sense, but it got by every defense on my computer.
I have added a few more add-ons to my Firefox in the security department and now have to figure out how it got on my computer.
The most likely suspect was an embedded video on site that installed a Flash Media Player, I think it was FLV, without permission. I uninstalled it immediately, but my problems happened shortly after that.

You'll have to identify what the "name" of the malware is. I'm sure you'll find plenty of instructions on completely removing it with a google search of the name.

Damn it is still here.
And BTW it imported the Paizo golem into it's tab next to the "By LoudMo".
I have cleared all the Cookies, Ran clean up several times, searched for the names on the add/survey, and reinstalled Firefox.
I am running out of ideas.
I also recommend running ad-aware as well as spybot search and destroy.

Well, just in case it's being extremely persistent - HijackThis is a good tool to run in report mode and let an expert analyse to see if it's squirrelled its way deep into your system. Otherwise I hope hogarth's link helps, but these type of programs just do not respect A/RP to get rid of them...

Malwarebytes is one of the best free malware removal tools on the net. I would DL both malwarebytes and spybot search and destroy. Should fix your problem.
I will second this... Malwarebytes is an amazing program. And it is free. One tip... If you wait to download it until you need it, you might be too late. it is so good that some malware will actually block you from downloading it! Seriously.
