Question 1: When a PC levels up, do you roll the hit dice he just gained and add that to his total HP? Or do you just add, for example, 10 HP if he just gained another d10?
Question 2: How does NPC/creature HP work? For example, does every rat or wolf have the same HP or do you roll and add their hit dice before the battle? For example, why does Kiramor the Forest Shadow have 39 HP when his hit dice are 4d10+2d8+6 (and what's that +6 from?). I thought maybe it was half of what you could roll with the hit dice, but that's not it either... I'm ridiculously confused...

1: your hit dice refers to the sided dice you roll to determine your extra hit points. so if you chose a level of fighter (d10) you roll 1d10 and take that result and add it to your hit point total.
2: NPCs work exactly the same way. For each level your roll one dice of the indicated type per level you get so Kiramor rolls a d10 4 times and a d8 2 times. The +6 bonus on the end comes from his Constitution score of 12. Your contitution bonus (+1) is added to every level you obtain. so since Kiramor is a 6th level character he adds his constitution bonus to his hit point total 6 times (1 x 6 = 6). If he had a constitution score of 14 (which grants a +2 bonus) he would add 12 hit points (2 x 6= 12)

Thanks for the explanation how the Kiramor character's scores add up. I was wondering the exact same question as OP (although 3 years later)
I got a question related to this thread:
I have the book GameMastery Guide and there are dozens of pre-generated NPC characters and as I was browsing through them I started to inspect their HP scores.
Military General (Fighter 11 lvl) has Con 14 and hp is 85 (11d10+25). Now, calculating with the formula of Con mod x 11 levels and you get +22, not +25. So there is a difference of 3 points.
The same goes with a 1st level Foot Soldier (human warrior, Con 11). He shouldn't with the formula in mind no +NN points to his total hp but according to the book he gets hp 8 (1d10+3). The same goes for 6th level Cavalry (there is a +3 added to the 6 additional points he gets with the Con mod).
However, a Caravan Guard (human fighter 2 lvl) with Con 14 gets hp 16 (2d10+5). Here the Con mod x levels should be 4 but instead is 5 points.
So my question (after this much rambling) is: is there a pattern for these additional points that are based on other things such as skills or feats that give a random amount of points to the hp (in addition to the Con mod x lvl)?

Two things to keep in mind:
1) Your first level gets you MAX HP. So, if your hit die is d10, and your con mod is +2, you will begin the game with 10 + 2 = 12 hp. This is to make sure no one starts the game with 1 or 2 hp. Every level after 1, you roll the die (and add con mod) instead of maxing it. (Some GMs prefer not rolling hit dice at all, in which case you get average hit points at every level after 1, so for example if your hit die is d10 and your con mod is +1 you will get 5.5 hp per level, so it would go: level 1, 10 hp; level 2, 15 hp; level 3, 16 hp; level 4, 21 hp; level 5, 27 hp, etc.)
2) For animals and monsters, it is common to always give them average hp per hit dice, and not max at level 1. So if there hit die is d8, they get a flat 4.5 hp/level.

I've got a question about increasing your hit dice? I read the toughness feat that you gain more HP as you gain hit dice (...when you level). So does that mean every time i level up I also gain a hit dice.
Example:
Level 1 fighter with 1 hd 10. When I hit level two is my hit dice now 2 hd 10?
I've looked in the core rule book but it's hard to determine what happens to your character after level 1. Thanks.

Lets say you are a level 1 fighter with a constitution score of 12. As a fighter you get 1 ten-sided dice of hit-points per level. And because you have a constitution score of 12 you gain an additional +1 at every level. If you had a con of 14 it would be +2, a 16 would be a +3, etc. In order to ensure that you don't die from a stubbed toe everyone gets their max hit-points at level one.
Every level after 1st your fighter gets to roll a ten-sided dice and add that to his hitpoints. then you add your constitution modifier in this case +1 to the total. The toughness feat adds 3 hitpoints to the total plus an extra hitpoint for every level beyond 4.
