Flat footed, Helpless, Stunned, Unconscious, Asleep, Dying, Inhaled poisons (might be scentless too and therefore "unnoticed"), Stilled silent spells from an undetected opponent, attacks from an apparent ally (dominanted ally possibly or just the garden variety traitor).
Good list, seems a bit generous. The ability works when "you are otherwise unware of an attack." Not when you are unable to defend yourself properly.
So I would agree with Unconscious and Asleep, and Inhaled(odorless)poisons or Ingested poisons, asumming you aren't consuming poisoned food or drink intentionally. And of course Dying makes you unconscious. The first attack from an apparent ally of course.
Stunned and even Helpless you are still aware of the attack(unless another condition makes you unaware).
There are many conditions under which you can be Flat-Footed but aware of an attack, the first round of normal combat before your initiative, for example.

I would actually lean towards also including "1st Normal Round of Combat before your Init is up".
...Beyond the arbitrary division of rounds, it essentially isn't any different than the Surprise Round (which the ability explicitly applies to from the perspective of the Surprised/Low Init character. Honestly, I don't think anybody is taking this Bloodline for it's uber-powered abilities, and I don't think such a minutely generous interpretation is going to change that. As well, allowing it to work before your Init is up seems over-all closest to the SPIRIT of the ability - This is something that would be nice to see Errata'd if such interpretation is indeed in-line with the design intent.| Dennis da Ogre |

I think I'd just house rule in the Diviner's much nicer school benefit rather than mess with this fairly goofy/ conditional bonus. Destined is not very powerful compared to the other bloodlines.

Bumping this to raise another question about the same ability: the wording is ambiguous as to when the bonus to saves applies. Should the Fated power be read as,
"During surprise rounds and when you are unaware of an attack, you gain a luck bonus to your saves and to your AC"
or as
"you gain a luck bonus to your saves and, during surprise rounds or when you are unaware of an attack, to your AC"?
