1)an arrow slips through a hole in a guy's quiver; and slips pointwise into the back of his britches; under and through his belt even...he's gotta waste a move action digging the arrow out from next to his bum or risk getting gigged a good one.

Hrmm... I would be a little careful with this. The mechanic is simply wasting their attack action already (or any action they have to roll for). To have it additionally cause them to loose other actions is to make this extremely powerful ability even more powerful.
I would really try and keep any negative effects to just described why they missed the roll that they just made. So with your example, it might be a good reason why someone missed a climb or balance check, but I wouldn't give any additional negatives for having had missed a roll that they had to make twice and take the worst roll.
All that said, I think it is a great idea to have a thread of reasons why rolls were missed. Maybe create a list of rolls that are likely to be made in the encounters and then come up with a few reasons for how unlucky they are in each situation.
