I want to add this to my subscription and my (perhaps erroneous) understanding was that one has options about which module is mailed first, but as soon as I add the subbie to my cart it says Pact Stone Pyramid and I want to start with Hungry are the Dead.
Is this option available? I'm confounded.

Unfortunately, I have nothing but bad news for you. :(
As it goes when starting all of our subscriptions, you can choose to start with either the most recent volume, or the next one. All volumes previous to the most recent are only available as a non-subscription purchase. Since we started shipping Pact Stone Pyramid last week, the previous volume, Hungry are the Dead, is no longer eligible to be a starting volume.
For your other question, while I technically can create the subscription for you, I really prefer not to. This is not an act of laziness on my part, but rather a way for me to make sure that all of the choices involved in the creation of a subscription (shipping address, payment method, combined shipping option, etc.) are in line with your wishes and not based on an assumption I have to make for you. I'm good, but not that good. :) Granted, I can nearly always make the correct assumption based on your choices for your other subscriptions, but this is not a guarantee. When it comes to your money and how you spend it, I prefer to avoid making assumptions.
